All Student Athletes must have a valid complete physical dated after April 1 prior to play/practice. Physical forms are available in the school office or on the school website . Completed Physical Includes: - Medical history - Physical (Dated and signed) - IHSAA Consents - Hendricks Consents - Concussion Acknowledgement Information - Sudden Cardiac Acknowledgement & Signature
