Eligibility Procedures

PHYSICAL EVALUATIONS AND FINAL FORMS To be eligible for the 2025-26 school year, your physical form must be dated on or after April 01, 2025. All forms, including the physical form, must be completed by June 02, 2025 in order for the athlete to participate in summer workouts or the student-athlete’s first workout, whichever occurs first. You must turn in your completed physical form to the athletic office. The physical form MUST be signed by the parent or guardian. The physical form MUST be signed by the student-athlete. The physical form MUST have the signature and license number of the physician. You must complete all forms on Final Forms. ALL forms on Final Forms are required to be completed. To access the forms on Final Forms: Select the Final Forms logo on the menu. Follow the directions on the Final Forms Parent Playbook. The link in the Playbook will send you to the correct page. What you should do after the EFFECTIVE DATE of April 1, 2025 Pick up the Pre-Participation Physical Evaluation Form from the athletic office, or download it from the menu. Complete the “History Form” on the Pre-Participation Physical Evaluation Form. Have your physician complete the “Physical Examination Form” on the Pre-Participation Physical Evaluation Form. The coach will not allow you to practice if you have not completed all of the above. All information is accessible to our coaching staff and no white cards are distributed to student-athletes. ATHLETIC FEE Noblesville High School has an Athletic Fee for all athletes participating in any sport. Noblesville High School implements an Athletic Fee for athletes participating in the athletic program. The fee covers transportation, technology, and Final Forms. It is $50 per athlete, per season, with no maximum per family during the school year. If you are on Free/Reduced Lunch, the fee is less. You may pay the fee by check or money order and turn it into the coach or the athletic office. At this time, we cannot offer an online payment option. In order for your child to continue participating, the Athletic Fee is expected to be paid prior to each season. If your student qualifies for free or reduced-priced meals, please bring in the letter you received stating your student’s lunch status. If you are not sure if your student qualifies for meal assistance, please call the Educational Services Center at (317) 773-3171. If you do not have this letter, you must complete and sign the form and return it to the Athletic Office or the Educational Services Center. The form is also available in the NHS Athletic Office. If you are on Reduced Lunch then you will pay a reduced fee. If you are unaware of your fee, please contact our office for assistance. Please direct any and all additional questions to the athletic office. If not, we will expect payment to be made in full by the deadline listed above.
