ATHLETIC PARTICIPATION REGISTRATION VIA FINALFORMS Below you will find the policies & procedures for various situations and activities that may be required or occur while participating in athletic activities at Central Catholic Jr./ Sr. High School. Online Student – Athlete Registration for Sports Participation We are very excited to announce that the Central Catholic Jr./Sr. High School Athletic Department has partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign athletic participation forms for your students. The most exciting news is that FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, saving you time. You may review your data at any time to verify it is current. You will be required to sign your forms once per year and after any update. ALL parents of athletes will use FinalForms to register your students for Athletic participation at Central Catholic Jr./ Sr. High School. IHSAA Pre-participation Physical and Health History Update Questionnaire and Consent and Release Certificate. All student-athletes are required each year to have a physical prior to participating in any Central Catholic sport-related activity (conditioning, open facility, weights, practice) by Monday, June 3, 2024.
