BC Parents and Guardians, Final Forms registration is now open for the upcoming school year! For returning athletes and their parents, simply log into your existing accounts and complete the required forms for the new school year. Both the parent and athlete must complete this process separately. If you are new to Final Forms, please follow the same signup process as last year using the attached Parent Playbook for guidance. When creating your child’s account, use an email they will remember, such as their school email. If your child is an incoming athlete without an active Benton Central student email, you may temporarily use the same email as your parent account or any other active email that is checked regularly. Steps to Complete Registration: Log into your Final Forms account and click the Big Blue Box. Click the "Ok, Let’s Get Started" button. Under "My Students," click the Blue Box labeled "Incomplete Forms". Select only the sports your athlete will be participating in for the upcoming school year, then click "Update Sports". Under the Registration Screen, complete all forms listed under "(Athlete’s Name) Forms". Verify that all information is accurate and up to date. Sign each form by typing your name in the box. Forms will turn green once signed. Forms that remain yellow indicate that your athlete still needs to sign them. Once all forms are signed and green, print the last form: "IHSAA PPE Physical (for Physicians)" and bring it to Athletic Physical Night along with the required payment.
