Tri-North Middle School

Tri-North Families, This coming school year the athletic departments at all three middle schools (Batchelor, Jackson Creek, and Tri North) will be making some improvements on how we will be collecting sports physicals for student athletes. We will be making a shift to a web-based system called FinalForms. All caretakers/families will be required to create an account on FinalForms and upload your student-athletes completed physical, prior to being eligible to participate in any conditioning or practice. Click on the Parent Playbook (attached) to get started in FinalForms. If you require any support during the process, scroll to the page bottom-right and click “Get Help”. From this point forward, you will not be allowed to hand in or email a physical to the front office, coaches, or athletic department. The physical must be uploaded to your FinalForms account. The physical form must be completed on or after April 1, 2023 to participate in any sporting activity for the 2023-2024 school year. Students will not be allowed to practice/play until the entire form is filled out. We are modeling Bloomington High School North's procedure and going completely paperless. Once your child graduates from Tri-North, they will follow the same guidelines at Bloomington High School North. Please follow the instructions from the Parent Playbook (attached) and select the FinalForms tab on the lefthand side of screen to create your account.
