In order to participate in a sport during the 2026-27 school year, each student athlete and parent will need to complete their portion of registration in FinalForms. Each student athlete will also need a completed IHSAA Pre-Participation Physical. The PPE Form must be signed by a physician (MD or DO), nurse practitioner or physician assistant holding a license to practice in the State of Indiana. This physical must be dated on or after April 1, 2026 in order to be valid for the 2026-27 school year. The physical and final forms registration must be completed before a student athlete may participate in practices. NO STUDENT MAY PRACTICE ON OR AFTER THE FIRST DAY OF PRACTICE WITHOUT ALL FINALFORMS COMPLETED AND A PRE PARTICIPATION PHYSICAL ON FILE - NO EXCEPTIONS!!!
