Perry Meridian High School Athletics

All Falcon Student-Athletes need an up-to-date physical to participate in athletics for the 2025-2026 school year. We have two options for discounted physicals. 1. Community Health Network, through our partnership with Community Clinic at Walgreens will offer discounted sports physicals for $30.00 at our Community Clinic at Walgreens Sites. This discounted rate is available during July 7, 2025, through July 20, 2025. An appointment must be made. Please see the attached information that you can send to families, post on websites, and advertise. The attachment includes important information and instructions for families. 2. Our Falcon Physical night is July 23rd, 6 pm to 8 pm for $20 at Perry Meridian High School. This event coincides with the club fair and incoming freshman parent meeting to provide greater access for our Falcons. Any Perry Township student-athlete in grades 6 through 12 can receive a sports physical during this opportunity.
