Perry Meridian High School Athletics

Any enrolled students in grades 9-12 are eligible to try out but must have a valid physical in order to try out. Physical Night is July 24th 6:00 - 8:00 pm at Perry Meridian High School (Enter Door #18) | Cost $20 Every student-athlete will need the Medical History Form completed prior to entering.
