We utilize Healthy Roster when it comes to submitting preseason paperwork for our Lumen Christi Athletes. The 2 forms required before participating in a one of our sponsored sports are the MHSAA physical form and the Henry Ford Jackson Sports Medicine Services Authorization Form. By having these forms submitted, approved, and stored on Healthy Roster you as the parent can… 1) make sure it is seen and received by the correct individuals. 2) always access the forms in the case of a need outside of LC sponsored sports. All enrolled students should have an account and an invitation to access the account was sent to each of the parent/guardian email(s) that are on file through FACTS. This process is completed every summer. If you have a newly enrolled student, a 6th grade student, or you still need an invitation, please follow the steps below.
